ACT Diversifies to Better Serve

 Mental Health Needs of Clients

 

ACT is aggressively reaching out to the mental health provider community during State Reform to identify mental health needs that are currently inadequately served.  We are partnering with Area Programs (Mental Health Centers) / Local Management Entities to fill gaps in quality care in the North Carolina Community.  Review our updated brochure below to see some of our new available services.

 

ACT Medical Group, PA is a Psychiatrist-owned Professional Association that serves the entire North Carolina State.  We have provided for the mental health care needs within long-term care facilities since 1996.  Our focus has been to provide comprehensive, quality care to those in need by mobilizing clinicians.  Serving individuals where they live allows our clinicians to holistically treat through incorporating all caregivers in improving quality of life for each individual served.

 

Services ACT provides include:

Evaluation

Consultation

Limited Testing

Psychotropic Medication Management

Psychotherapy

Coordination with Discharge Planners for referral acceptance

Emergency Care

ACT’s Medical Department Manager (MDM), an RN, CRC, LPC is available by pager to triage your need.  ACT Psychiatrists are available to the MDM for needs that necessitate psychiatrist involvement.  The ACT MDM has authority to schedule emergency visits that will occur no later than 24 hours post-call.  The MDM additionally coordinates with Private Inpatient Units, Emergency Rooms and Crisis Centers when the emergency is immediate.

Facility Staff Training

ACT clinicians are approved as Continuing Education providers by the Division of Facility Services.  ACT has a series of inservices currently available to our facilities and we are happy to speak to the need.

 

ACT regularly lobbies insurance companies and policy makers at the local, state, and federal level to improve upon accessibility to health care for individuals in need.

Service Enhancements:

In 2000, ACT expanded service provision to new locations including hospitals, facilities for children and schools.

 

ACT is proud to announce expansion of services made possible through the Division of Mental Health’s support of Geriatric Specialty Teams.  ACT’s Geriatric Specialty Teams will assist in locating appropriate placement for individuals currently residing in state institutions, successfully moving these individuals to community-based residences and actively creating and supporting infrastructure to divert individuals from the need to utilize state institutions in the future.

 

For Area Programs/ Local Management Entities that contract with ACT Medical Group, PA, the following services will now be added to current services available for facilities within the Area Program’s service area and to consumers referred to ACT by the Area Program and/ or State Institutions:

Case Management

Ø      ACT will assess nursing home and adult care home strengths and weaknesses for individual discharge planning

Ø      ACT will assist referred individuals in obtaining needed resources such as entitlements, legal assistance and medications

Ø      ACT will assist with referrals based on client choice to community-based mental health services

Psychosocial Activities

Ø      ACT will assist with transition of individuals from state institutions to facilities

Ø      ACT will assist staff in assessing behaviors and developing and implementing intervention plans

Ø      ACT will provide for family education

Generic Technical Assistance

Ø      Staff training currently available to facilities by ACT will now be State-funded through the Geriatric Specialty Team.  ACT training available will expand to include new staff orientation on topics such as individual rights, identification of ongoing training resources for staff and provision of resource information to staff relevant to the population.

Ø      ACT will increase community opportunities for individuals through establishment of linkages with community providers serving the geriatric population and will additionally coordinate support and assistance from state hospital staff who have geriatric expertise.

Medical Department Manager

Registered Nurse, Certified Rehabilitation Counselor, Licensed Professional Counselor

Manages emergency response; facilitates communication needs between providers, consumers and caregivers; assures that psychiatry training and supervision is scheduled as appropriate.

 

Professional Relations Director

Oversees activities of Professional Relations staff in their function of ensuring consumer satisfaction and educating facilities on mental health and ACT functions.

 

Administrative Services Manager

Oversees corporate office staff operations to include staff scheduling, billing, medical records functions, recruiting, credentialing and financial operations.
