A Note from the President
ACT
Medical Group, PA touched the lives of approximately 7,000
individuals in 2003. Most
of these persons live in Long-Term Care Facilities although some
were patients in hospitals. I want to express my appreciation for:
Our
clinicians, who invested their time, energy and caring into
assisting individuals regain quality of life
Our
support staff, that worked diligently to minimize administrative
hassles for our clinicians so that they could concentrate on what is
most important- care
Our
patients’ caregivers, including families, facility staff and
primary care physicians, for working as team members with us which
is crucial to long-term success
Our
patients, for entrusting us with their care
Please

ACT’s Comprehensive Care Promise
Medical
Personnel within ACT’s Clinical Services Department consist of
Psychiatrists, Certified Nurse Practitioners, Certified Physician
Assistants and a Clinical Support Manager.
These experienced and licensed professionals, in
collaboration with our clinicians providing psychological care,
provide comprehensive mental health care to individuals served by
ACT. ACT staff assists
patients in dealing more effectively with losses, frustrations and
various difficulties arising from medical and emotional situations.
Timely
and comprehensive visits are arranged in the individual’s living
environment to assess the needs of the patient, interact with family
members and coordinate care with Primary Physicians and caregivers.
Medications are monitored closely for effectiveness and
potential drug interactions. Pharmacy
recommendations as well as lab studies are reviewed as a measure of
continued quality service.
All
persons yearn for health and happiness.
Unfortunately circumstances arise which deprive us of these
desired goals. ACT
strives to reduce each individual’s distress in his or her own
surroundings. However,
if hospitalization is required, our clinicians will facilitate
confidential and appropriate care.
An after-hours emergency on-call service is available to
handle the needs of our patients.
We
at ACT will continue to advocate for improved mental health care in
facilities we serve, in our communities and across North Carolina.
Author of this article, Jean Patel, RN, CRC,
LPC serves as ACT’s Clinical Support Manager and is responsible
for ensuring appropriate communication between ACT clinicians and
other caregivers for patients under our care.
Mrs. Patel has 20+ years of mental health experience both
inpatient and outpatient.
