ACT Medical Group provides primary care and mental health (psychotherapy and psychiatric) care for long term care patients located in nursing homes and assisted living communities. Through mobile primary care physicians, psychiatrists, psychologists and medical professionals that we employ, care is delivered directly to adults and geriatrics. ACT also offers on-site services to agencies with clients who need us.
By serving individuals in their familiar setting, ACT clinicians are able to
fully assess presenting problems and to incorporate active caregivers into
plans for improving quality of life. Our preventative care, care oversight,
medication management and various forms of therapy are all conducted with
the mission of providing the most effective and least intrusive care
available to the individual served. Our ongoing education offered to the
community supports these values.
